The wire/nylon cord passes through the top, and out the bottom of the hook. It has a spring loaded tightener that automatically tightens on the inserted cable. Pressing the cylinder sticking out on the top (through which the cable fits) releases the hook. Although this might seem a bit fiddly it is actually fine. There is a hole near the end of the hook that can be used for tying it to the fixing (the makes adjusting the hook a lot easier, and is almost like a security hook). It is fairly small in diameter, although not quite the smallest on the market. All in all, a good hook, well engineered and thought out, at a good price.
